Referrals are only accepted from licensed Canadian physicians

Please include any relevant information, including past treatments and investigations. Do not refer patients that are currently being investigated for RED FLAG pain signs and symptoms as these will be returned until appropriate investigations are completed.

Please note that we also accept referrals for patients with ongoing Workers' Compensation Board (WCB) claims.
